Documentation to be sent to IHSA after completion of a Working at Heights course:
1) IHSA - Participant Information and Consent form
2) IHSA - Answer Sheet
3) IHSA - Participant Course Evaluation
4) IHSA - Answer Sheet
5) IHSA - Participant Course Evaluation

IHSA must receive all completed WAH paperwork within 3 business days from your training date in order for us to meet the 7 business day turnaround with Ministry of Labour.
