Risk Assessment and Root Cause Analysis workshops

Created for and by the industries IHSA serves, our risk assessment and root cause analysis workshops are designed to help high-risk sectors identify potential hazards and assess the likelihood of harm. In collaboration with the Ministry of Labour, Immigration, Training and Skills Development (MLITSD), IHSA works alongside employers, workers, academics, enforcement, and IHSA staff to deliver these workshops. After completion, IHSA shares the findings and resources with industries across the province, supporting organizations in improving safety.

Risk Assessments: A risk assessment is the process of identifying potential hazards that currently exist or may arise in the workplace, helping to evaluate the risk and determine necessary precautions.

Root Cause Workshops: A root cause workshop is a structured, collaborative session aimed at identifying and analyzing the underlying causes of an issue. The goal is to develop effective solutions that address the root cause and prevent future occurrences.

Completed Workshops

Driver Fatigue (General Trucking): Fatigue is a contributing factor in thousands of crashes annually. This section provides a range of resources, including insights from the 2019 Driver Fatigue Root Cause Workshop within the General Freight Trucking Sector.

Slips, Trips, and Falls (Surface Mining and Aggregates) : Beginning with a 2014 risk assessment for the Surface Mining Sector and followed by a 2023 root cause workshop on slips, trips, and falls facilitated by IHSA, this section offers resources to support safety improvements. Included are the full workshop results, the technical report, and resources to promote best practices for preventing slips, trips, and falls in the surface mining and aggregates sector.

Underground Strikes (Utilities): Due to the high frequency of underground utility strikes, IHSA hosted a Root Cause Workshop in 2021. This section includes the full workshop results, a technical report, and resources to support best practices for working with underground utilities.

Working at Heights (Residential Roofing): In 2015, IHSA, in partnership with MLITSD, conducted a risk assessment for residential roofing – Working at Heights. This section includes the results of the risk assessment, as well as the individual root cause workshop findings for Rough Framing/Carpentry Trades, Homebuilding Trades, Masonry and Bricklayer Trades, Roof Shingling Trades, and Siding and Exterior Finishing Trades.

Yard Safety (General Trucking): Trucking yards are busy environments with numerous moving parts, including pedestrians, commercial vehicles moving in various directions, and other vehicles entering or exiting the yard. In 2021, IHSA facilitated a Root Cause Workshop within the General Trucking Sector. This section offers access to the full workshop results, a technical report, and resources to help support best practices in trucking yard safety.
