Before any worker enters a confined space or performs related work, a Confined Space Entry Permit must be completed. The permit identifies the work to be done and lets workers know that the hazards have been identified and the controls are in place.

This permit is in a pad format. Each pad contains 50 white originals and yellow duplicates. 8½" x 11". Rev. May 2014.
